Crisis OutReach

For individuals and families who have received crisis services, OutReach staff will work with them for a limited period of time (up to 30 days following a crisis assessment) to help ensure that the crisis has been resolved.

OutReach staff can:

  •     Assist you with the transition from a hospital or crisis unit.
  •     Provide information and referrals to other community services.
  •     Provide support by phone or in person.
  •     Work with you and your other providers on a safety plan and ways to avoid future crisis situations.
